If you have commercial insurance through your employer, your claims and payments will be processed securely through Headway.

Identification & Insurance Requirements
↳ To protect patient safety and comply with regulations, the following are required at the time of booking
• A clear photo of your insurance card (for insured patients)
• A valid photo ID for all patientsIf you are seeking controlled medications, photo ID upload is mandatory
Appointments will not be accepted without:
• A photo ID
• An insurance card (if using insurance)
Exception: Patients paying privately are not required to upload an insurance card.
Intake Forms
All intake forms must be:
Incomplete paperwork may result in cancellation or rescheduling.
Completing forms ahead of time allows us to focus fully on your care during your visit.
No-Show & False Insurance Policy
A $75 no-show fee will be charged to
• Any patient who does not attend their scheduled appointment
• Any patient who books using false or invalid insurance information
We value your time and ask that you respect ours as well. If you need to cancel or reschedule, please notify us in advance.
